Probate Forms (MA)

There are a number of state-specific forms you may need when going through the probate process.

General MA Forms

In Massachusetts, the local Probate and Family Court handles wills and estate probate.

Below you will find links that allow you to download and use some of the more common MA probate forms (you may want to check with your local court for form preferences):

  1. Notice of Intended Probate (Form MPC-550) - Notify heirs that you plan to begin probate
  2. Petition for Informal Probate (Form MPC-150) - Get the probate process started
  3. Heirs-at-Law (Form MPC 162) - Include with petition the list of heirs-at-law
  4. Devisees (Form MPC 163) - Include with petition the list of people named to inherit via the will
  5. Military Affidavit - Include with petition to declare decedent's military status (or non-status)
  6. Will Witness Affidavit (Form MPC-480) - Include with Petition if there is a will but it is not self-proving
  7. Waiver (Form MPC-455) - Include with Petition for heirs to waive bond and/or executorship
  8. Bond (Form MPC-801) - Include with Petition if bond not waived
  9. Order of Informal Appointment (Form MPC-750) - Include with petition a pre-filled form for the judge to sign
  10. Notice of Probate (Form MPC-551) - Notify creditors that you have begun the settlement process
  11. Inventory (Form MPC-854) - Tell the court about estate contents
  12. Final Accounting (Form MPC-853) - Submit final financials to court
  13. Petition for Complete Settlement (Form MPC-855) - Ask permission from court to complete settlement
  14. Closing Statement (Form MPC-850) - Submit Closing Statement to court
