General Forms
In Wisconsin, the local Circuit Court handles wills and estate matters.
You can check with your local court for the forms they might prefer, and below you will find some of the more common WI forms:
- Application for Informal Administration (Form PR-1801) - Get the probate process started
- Waiver and Consent to Informal Administration (Form PR-1803) - Get permission from heirs
- Notice of Hearing (Form PR-1805) - Notify anyone named in the will, and anyone who would normally inherit
- Estate Inventory (Form PR-1811) - Report estate contents (consider attaching EstateExec Inventory Report)
- Creditor Notice (Form PR-1804) - Send notice to creditors
- Estate Accounting (Form PR-1814) - Tell court about estate finances (consider attaching EstateExec Final Accounting Report
- Estate Receipt (Form PR-1815) - Get receipt for distribution or debt payment
- Closing Statement (Form PR-1816) - Tell the court you are finished with your duties
