MI Probate Forms

There are a number of state-specific forms you may need when going through the probate process.

General Forms

In Michigan, the county Probate Court handles wills and estate matters.

You can check with your local court for the forms they might prefer, and below you will find some of the more common MI forms:

Application for Informal Probate (Form PC-558) - Get the probate process started
Acceptance of Appointment (Form PC-571) - Tell the court you accept the executor appointment as
Notice to Creditors (Form PC-574) - Publicly notify creditors of their time limit
Notice to Known Creditors (Form PC-578) - Individually notify creditors of their time limit
Proof of Service (Form PC-564) - Tell the court you have made the required notices
Inventory (Form PC-577) - List estate contents for the court
Schedule of Distributions and Payments of Claims (Form PC-596) - Tell the court how you intend to settle the estate
Close Administration (Form PC-591) - Submit Closing Statement
Certificate of Completion (Form PC-592) - Tell the court everything is done
