Probate Forms (ME)
There are a number of state-specific forms you may need when going through the probate process.
This page lists common ME forms and offers help in locating others. While you are welcome to browse these forms, note that the Tasks Tab will guide you through the use of relevant forms at the appropriate times.
General ME Forms
In Maine, the County Probate Court handles wills and estate probate.
Below you will find links that allow you to download and use some of the more common ME probate forms (you may want to check with your local court for form preferences):
- Application for Informal Probate (Form DE-201(I)) - Get the probate process started
- Application for Informal Appointment - Intestate (Form DE-101(I)) - Get the probate process started if there is no will
- Renunciation (Form DE-407) - Include in your application, from anyone with equal or higher priority
- Certificate of Value (Form DE-401(A)) - Include estate value estimate in your application
- Acceptance of Appointment (Form DE-104) - Include in your application
- Waiver of Notice (Form N-107) - Allow interested parties to waive notice requirements
- Notice of Appointment (Form N-115) - Notify interested parties of your appointment as executor
- Inventory (Form DE-405) - Report estate contents
- Probate Accounting (Form DE-406) - Report estate financials
- Petition for Complete Settlement (Form DE-601) - Ask court for permission to distribute
- Verified Application for Certificate of Discharge (Form DE-605) - Tell the court you are finished with your duties
