OH Probate Forms

There are a number of state-specific forms you may need when going through the probate process.

General Forms

In Ohio, the Probate Division of the local Court of Common Pleas handles wills and estate matters.

You can check with your local court for the forms they might prefer, and below you will find some of the more common OH forms:

Application to Probate Will (Form 2.0) - Get probate started (also includes optional Waiver of Notice and Certificate of Waiver)
Application to Administer Estate (Form 4.0) - Get probate started if there is no will
Notice of Probate (Form 2.2) - Send to people named in the will and heirs who would inherit if there were no will
Certificate of Notice Service (Form 2.4) - Tell the court you have made the required notices
Inventory (Form 6.0) - Summarize estate contents for the court
Asset Schedule (Form 6.1) - Detailed asset list for the court
Fiduciary Account (Form 13.0) - Submit Final Accounting to court
Receipts and Disbursements (Form 13.1) - Detailed attachment for Fiduciary Account
Certificate of Termination (Form 13.6) - Inform court that estate has been settled
