Probate Forms (MB)
There are a number of province-specific forms you may need when going through the probate process.
This page lists common MB forms and offers help in locating others. While you are welcome to browse these forms, note that the Tasks Tab will guide you through the use of relevant forms at the appropriate times.
General MB Forms
In Manitoba, the Probate Division of the Court of Queen's Bench handles probate and estate administration (see Court of Queen's Bench locations).
Below you will find links that allow you to download and use some of the more common MB probate forms (you may want to check with your local court for form preferences):
Initial application:
- Request for Probate (Form 74A) - Get the probate process started if there is a will
- Affidavit of Will (Form 74D) - Include Will with Request for Probate
- Grant of Probate (Form 74F) - Include with Request as a courtesy to court
- Request for Administration with Will Annexed (Form 74C) - Get the probate process started if there is a will but you are not named executor
- Letters of Administration with Will Annexed (Form 74G) - Include with Request as a courtesy to court
- Request for Administration (Form 74L) - Get the probate process started if there is no will
- Letters of Administration (Form 74Q) - Include with Request as a courtesy to court
- Nomination (Form 74M) - Include with Request for anyone with higher executor priority
- Bond Waiver (Sample) - Request to waive probate bond
- Affidavit of Bond Execution (Form 74T) - Tell the court you obtained any required bond
- Inventory and Valuation (Form 74B) - Preliminary estate inventory
Subsequent forms:
- Application to Pass Accounts (Form 74Z) - Ask the court to approve final estate accounts
- Affidavit Verifying Notice and Accounts (Form 74AA) - Swear that the accounts are correct

Small estates sometimes need only 1 specialized form or affidavit: see Small MB Estates.
Resealing
If the estate needs to probate real estate in another province, you may be able to reseal the original grant of probate rather than start from scratch in the secondary province: see Ancillary Probate & Resealing.
